Fire risk assessment template. A printable fire risk assessment form, covering the premises, the people at risk, the hazards, the fire safety measures already in place and the significant findings that come out of it.

This is the paper version. FireOptix does the same job on a phone, with the record filed against the building or the asset automatically.

What is on the sheet

  • Premises details, use and occupancy, including sleeping accommodation and out-of-hours use
  • People at risk, with space for those who would need help to escape rather than a single headcount
  • Fire hazards grouped as sources of ignition, fuel and oxygen, which is how the assessment is conventionally worked through
  • The fire safety measures already in place — detection and warning, escape routes, lighting, signage, firefighting equipment and compartmentation
  • A significant findings block, because the findings are the part the Order is interested in
  • An action plan with a priority, an owner and a completion date against every item, and a review record at the end

Why this record matters

The Regulatory Reform (Fire Safety) Order 2005 requires the responsible person to make a suitable and sufficient assessment of the risk to relevant persons, and — in the circumstances the Order and the Fire Safety (England) Regulations 2022 set out — to record the significant findings. A form cannot make an assessment suitable and sufficient. What it can do is stop the findings being the part that never gets written down, which is the failure an inspecting officer meets most often.

The structure here is the conventional one: look at the premises, look at who is in it, look at what could start a fire and what would feed it, look at what is already in place, then say what you found and what you are going to do about it. The hazard groupings are a working convention rather than anything prescribed, and the measures list is a prompt rather than a specification.

The half most forms leave off is the end of it. An assessment that identifies a blocked escape route and stops there has recorded a problem and nothing about the response — and a finding with no owner and no date is the one that is still open at the next inspection. The action plan on this one runs to a completion signature, and the review block records when the assessment was next looked at and what changed.

Who may carry this out is a separate question from what the form asks. The Order requires the assessment to be made by a competent person, and for anything beyond a simple premises that is not usually the person who prints the form off. This sheet is a record of an assessment, not a qualification to make one.
